VR for Convergence Insufficiency: What Studies Show

Office vergence therapy for convergence insufficiency: what CITT found

Convergence insufficiency (CI) means the eyes have trouble turning in together for close work, which can cause eye strain, headaches, blur or double vision up close. Our guide to convergence insufficiency exercises covers diagnosis and the non-digital options.

One large children's trial is the Convergence Insufficiency Treatment Trial (CITT), published in 2008. In 221 children aged 9 to 17, 73% of those who had weekly office therapy with home practice were successful or improved after 12 weeks. The figure was 43% with home pencil push-ups, 33% with a home computer program and 35% with placebo office therapy. A 2020 Cochrane review found high-certainty evidence that, in children, office therapy beat home computer therapy, and it called the adult evidence less clear. In adults, the CINAPS trial found office therapy improved eye-teaming measures more than placebo, while symptoms improved in both groups. Our guide to convergence insufficiency games covers the home computer trials.

How VR creates near and far vergence demand without a Brock string

With a Brock string, the beads sit at real distances, so your eyes turn in and your focus changes together as you look from far to near. A headset has to fake distance. It does this in three ways:

  • Disparity: the headset shifts each eye's image sideways. Moving the two images toward the nose makes the eyes turn in to keep them single (convergence); moving them apart makes the eyes turn out (divergence). Boon 2020 changed the separation between the two images inside a game.
  • Virtual prism: programs often express that shift in prism dioptres, the same unit as a clinic prism bar. In Li 2022, demand ran from 20 base-in to 30 base-out and ramped up by 2 prism dioptres a second, and the person reported when the target doubled or a suppression cue for one eye disappeared.
  • Virtual distance: a 3D scene can place an object close to your face. In Yang 2025, game targets came to about 10 cm in front of the eyes to draw them in.

What VR can't copy: focus and checking the eyes

In most headsets, the screen's focal distance stays fixed while the vergence demand changes. The 2026 Martinez-Perez review called this vergence-accommodation conflict "a structural limitation" and said it may reduce how much VR training carries over to everyday viewing. In Li 2022, among participants with accommodative dysfunction, VR improved focusing flexibility less than office therapy did, and the authors suggested adding variable lenses to headsets.

Clinic therapy also relies on a therapist watching your eyes. Most VR programs in the trials used no eye tracking and relied on what the person reported. VERVE's 2024 FDA summary describes a headset with built-in eye tracking. Eye care professionals can read our clinician articles on the near point of convergence test and fusional vergence ranges.

The VR vergence studies

Here is what each study found, as its authors reported. None enrolled children under 10.

What the reviews concluded

Islam and Roy's 2025 review in the Journal of Optometry found three VR studies with 60 people, whose average ages ranged from 18 to 26. The near point improved by 3.38 cm on average from before to after therapy. The authors said small samples and varied methods made the evidence insufficient for firm conclusions.

The 2026 Martinez-Perez meta-analysis included only randomized trials. For CI, those were Boon 2020 and Li 2022. Digital and VR therapy reduced the eye drift up close by 1.64 prism dioptres but had no significant effect on the near point or positive fusional vergence. It rated the evidence low to moderate and called VR a possible add-on for selected patients.

Other digital tools are not VR. A 2026 trial of a phone app that tracked the eyes with the camera found higher recorded adherence than pencil push-ups (90.1% vs 75.4%), but no clear difference in symptoms. Outside CI, a 2021 study found that one 25-minute VR gaming session improved focusing and vergence flexibility in adults without symptoms who had slow focusing or vergence, and its authors called the study small, unmasked and unrandomised.

Where VR stands next to CITT, and what's FDA cleared

No large trial has compared a home VR program with office therapy, and no VR trial has enrolled children of CITT's ages at CITT's scale. The one randomized VR-versus-office comparison, Li 2022, ran VR in the office with 33 adults and had no placebo group. Its participants started with an average near point of about 6 to 6.6 cm. CITT and the VR studies also differed in age, setting and how success was defined.

As of October 2026, the FDA's 510(k) database lists one cleared device for CI. VERVE, from OculoMotor Technologies, was found substantially equivalent through the 510(k) pathway on June 13, 2024 (K232930, Class I). It is for prescription use in people aged 11 to 35 under a medical professional's supervision. The clinical study in its 510(k) summary had no comparison group. A sham-controlled VERVE trial in adults with concussion-related CI is recruiting, with primary completion estimated for June 2027. Check availability and pricing with the company.

AAPOS says home treatment is often enough for CI and that no option has been shown to be best. The AAO says your doctor may prescribe office exercises, home exercises or both. Ask whether VR fits your plan, how you'll be retested, and what to do about headaches or double vision. Sudden double vision needs an urgent appointment.

Where Optics Trainer fits

Optics Trainer runs on Meta Quest, with the same games and drills as the mobile app plus VR-only activities. Its VR settings, listed below, use the same kinds of technique described above.

How the settings work: you can adjust contrast, brightness and blur for each eye yourself. Vergence levels come from the app's own in-game testing: the daily workout includes gamified tests of your vergence range and builds the workout from your results, which are practice results, not clinical measurements. Horizontal, vertical and torsional offsets are set in a calibration step, and the app then tests that you can fuse and see clearly at the values entered, so they cannot be set to arbitrary values. If you have a specific eye condition, use these settings with your eye care professional's guidance; Optics Trainer does not give treatment guidance.

Optics Trainer was not part of any study here and is not VERVE or any cleared device. It is practice, not a treatment or diagnosis, and it is not FDA cleared. It is not an alternative to office therapy or to exercises your clinician prescribes. Which settings to use, if any, is a decision for your eye care professional. Clinicians who need measurement can see Optics Tracker on our clinic site. Optics Trainer is not a substitute for professional diagnosis or prescribed care. Consult your eye care professional for guidance about your specific condition.

As of October 2026, the Meta Quest app is $19.99 a month or $199 a year after a 7-day free trial. The same games run on phone, tablet and web with red-blue glasses for $14.99 a month or $149.99 a year, with a 7-day free trial on phone and tablet and 3 free plays a day on web.

  • Vergence settings for convergence, divergence and virtual prism, the same kind of disparity-based demand the VR studies used.
  • Odd One Out, Front or Back and Motor Fusion in Convergence and Divergence versions, described in our guide to vergence exercises. Some show a vergence result, which is a practice result, not a clinical measurement.
  • Binocular (MFBF) mode, where each eye gets its own target while both stay open, with per-eye contrast, brightness and blur.
  • Peripheral fusion activities, where offset background objects show depth only once you fuse them.

Frequently asked questions

Does VR work for convergence insufficiency?

Studies of 9 to 33 young adults found the near point and fusional vergence improved after VR therapy. In the only randomized comparison with office therapy, a pilot in 33 adults found no significant difference between groups in symptoms or eye-teaming measures. A 2026 meta-analysis found eye drift up close fell by 1.64 prism dioptres, with no significant effect on the near point or fusional vergence.

Is VR as good as office vision therapy for convergence insufficiency?

We don't know yet. No large trial has compared home VR with office therapy. A pilot trial in 33 adults found no significant difference, but the VR sessions were done in the office and there was no placebo group.

Is there an FDA cleared VR treatment for convergence insufficiency?

Yes. As of October 2026, VERVE was cleared through a 510(k) in June 2024 for prescription use in people aged 11 to 35 under a medical professional's supervision. The clinical study in its 510(k) summary had 25 people and no comparison group.

How does VR train convergence without a Brock string?

It shifts each eye's image sideways so your eyes must turn in or out to keep one picture, often stepped in prism dioptres, or it places an object close to you in a 3D scene. Unlike a real target, the headset's focal distance usually stays fixed.

Can VR cause eye strain?

It can. The mismatch between fixed focus and changing vergence in headsets can reduce comfort, according to a 2026 review. Meta's warnings advise breaks and seeing a doctor first if you have binocular vision problems. Stop if you get eye pain, headaches or double vision.

Can kids use VR for convergence insufficiency?

Most VR CI studies were in adults. VERVE is cleared from age 11, under a medical professional's supervision. As of October 2026, Meta sets a minimum Quest account age of 10.
