Eye Exercises for Double Vision: Causes and the Studies

When double vision is an emergency

Double vision can be a sign of a stroke or another serious problem. MedlinePlus, from the US National Library of Medicine, says to seek emergency care if you have double vision, even if it goes away. Call 911 or go to the emergency room right away if double vision:

  • Starts suddenly with weakness or numbness in the face, arm or leg, trouble speaking, confusion or loss of balance. The CDC lists these, along with sudden trouble seeing, as stroke signs and says to call 911 right away.
  • Comes with a drooping eyelid, one pupil larger than the other, or a severe headache. A 2026 review by eye and nerve specialists says double vision with a droopy lid can come from a third nerve palsy. With a sudden severe headache, the review says the cause is likely an aneurysm, and the person should go to the emergency room at once.
  • Follows a head injury. The NHS lists double vision after a head injury, or with a severe headache or enlarged pupil, as reasons to call 999 or go to A&E, the UK's emergency services.
  • Comes with eye pain. MedlinePlus says any double vision needs emergency care. The NHS lists eye pain with double vision, and sudden double vision, as reasons for urgent same-day help.

Monocular vs binocular double vision

An American Academy of Ophthalmology (AAO) article for eye doctors sorts double vision by one question: does the second image go away when one eye is covered?

Binocular double vision happens only with both eyes open. It goes away when either eye is covered, because the eyes are not aimed at the same spot. The AAO article notes it can reflect a problem inside the skull. A clinician confirms this with tests such as the cover test as part of a binocular vision assessment.

Monocular double vision stays when the other eye is covered. It comes from the optics of one eye. The NHS lists dry eyes, astigmatism, cataracts and keratoconus as typical causes. A 2026 review in Deutsches Ärzteblatt International called it not dangerous. The AAO article says monocular double vision is usually refractive, meaning it comes from how the eye focuses light, so treatment targets that eye, for example with glasses, contact lenses or surgery.

What causes binocular double vision?

That 2026 review estimated that decompensated latent strabismus and cranial nerve palsies together account for about 70% of cases. Orbital, brainstem and cerebellar diseases come next. The authors noted that no population-based studies exist on this. Common causes include:

  • Convergence insufficiency (CI): the eyes have trouble turning in together for close work, which can cause blur or double vision up close.
  • Decompensated phoria: most people have a small hidden eye drift that the brain holds in check. The review says symptoms appear when that control fails, for example with fatigue or poorer vision.
  • Strabismus, an eye turn: the NHS says binocular double vision is usually linked to a squint and in adults can signal a more serious condition.
  • Cranial nerve palsies: weakness of the third, fourth or sixth nerve, which move the eyes. The AAO lists high blood pressure, diabetes and smoking as risk factors for the microvascular type.
  • Thyroid eye disease and myasthenia gravis, which affect the eye muscles or the nerve-muscle signal. The AAO notes myasthenia can also cause a drooping eyelid.
  • Concussion, which can lead to convergence insufficiency.

Eye exercises for double vision from convergence insufficiency

In the 2008 Convergence Insufficiency Treatment Trial (CITT) of 221 children aged 9 to 17, 73% of children in weekly office therapy with home practice were successful or improved after 12 weeks. That compared with 43% for home pencil push-ups, 33% for home computer therapy and 35% for placebo office therapy. A 2020 Cochrane review called the adult evidence "less clear." Our guides to convergence insufficiency exercises and convergence insufficiency games cover these trials in detail.

For CI after a concussion, the 2026 CONCUSS trial enrolled people aged 11 to 25 who were 4 to 24 weeks past their injury. After six weeks of twice-weekly office therapy, 88% (46 of 52) were successful or improved on eye-teaming measures, compared with 8% (4 of 52) of those waiting to start. Eye care professionals can read our clinician article on eye movements after concussion.

Programs for CI often use tools such as the Brock string and vergence cards, set by the clinician. Clinicians check progress with the near point of convergence and fusional vergence ranges.

Double vision therapy for eye turns and phorias

For other eye-teaming causes, the studies are smaller.

  • Intermittent exotropia, an outward drift that comes and goes: AAPOS says "Exercises have only been proven to work with convergence insufficiency." A 2024 trial of 40 children found office vergence and anti-suppression therapy improved control at 17 weeks compared with observation. A 2026 randomized trial of 177 children aged 6 to 18 found a home VR program improved distance control at 12 weeks compared with observation, by 0.33 points between groups, which the authors called modest. Children who did more than 75% of their training improved more. Our guide to strabismus exercises covers these.
  • Acute acquired comitant esotropia, a sudden inward turn: a 2026 retrospective study of 14 patients reported that 5 with smaller turns were relieved by vision therapy alone. The other 9 had prisms plus therapy. Of those, 2 reached single vision without prisms, 4 needed less prism power, 3 still needed full prisms, and 1 chose surgery.
  • Decompensated phoria: we did not find a controlled trial of exercises for this cause alone. In prism studies, described below, patients with decompensated strabismus often had their double vision resolved with prisms.

Prisms and Fresnel prisms for double vision: what studies found

A prism in glasses bends light so the two images line up. The AAO describes a Fresnel prism as a temporary stick-on prism for the lens of your glasses. It says this is not ideal long term but lets you try a prism first. It notes a prism does not help every type.

The 2024 Portela-Camino study in PeerJ tested prisms followed by vision therapy. It followed 48 adults with an average age of about 62, most with fourth or sixth nerve weakness. Prisms alone succeeded in 22 of 43 who finished. Of the 20 for whom prisms failed and who then did vision therapy with their prisms, in the clinic and at home, 13 regained binocular vision and reported better reading and daily function. The study had no control group, which the authors listed as a limitation. Two authors helped found VisionaryTool, the company that makes one of the two vergence programs used.

Nerve palsy and other causes: what treatment pages list

For sixth nerve palsy, EyeWiki, the AAO's clinical wiki, lists watching and waiting, a patch or Bangerter filter, prisms (which it says are often not useful in this palsy), Botox injections and surgery once measurements have been stable for at least 6 months. It says most microvascular cases recover within 3 to 6 months, and it does not mention exercises. The AAO's page on microvascular cranial nerve palsy says it almost always gets better on its own in about 6 to 12 weeks. It lists a patch, prism glasses and eye muscle surgery. In the Portela-Camino study above, palsy patients did therapy only after prisms alone did not work.

Myasthenia gravis is treated with medicines and sometimes surgery, the AAO says. The 2026 review describes thyroid eye disease as an autoimmune disease that thickens the eye muscles. The NHS notes that a squint does not always cause double vision; in young children it can cause a lazy eye instead. Our guide to visual suppression explains this, and our guide to binocular vision and eye teaming covers how the two eyes work together.

Where Optics Trainer fits

Optics Trainer is a set of game-based vision and brain training apps for iPhone, iPad, Android, web browsers and Meta Quest. It includes eye teaming games with convergence and divergence levels, binocular games where each eye gets its own target, and watch-and-follow drills with no score.

It was not part of the studies described above. It is not a treatment or diagnosis for double vision, it is not FDA cleared, and it does not measure anything. Optics Tracker, on our clinic site, is our measurement product. Do not use any app for new double vision before you have been examined. If headaches, eye pain or double vision appear during an exercise, stop and call your clinician. Optics Trainer is not a substitute for professional diagnosis or prescribed care. Consult your eye care professional for guidance about your specific condition.

As of October 2026, mobile and web training is $14.99 a month or $149.99 a year. The phone and tablet apps start with a 7-day free trial, and a free web account includes 3 plays a day. VR training on Meta Quest is $19.99 a month or $199 a year after a 7-day free trial.

Frequently asked questions

Can eye exercises fix double vision?

It depends on the cause. In trials of convergence insufficiency, office-based exercise therapy had the highest success rates. The AAO and EyeWiki pages on nerve palsy and myasthenia list prisms, patching, medicine or surgery, not exercises. Monocular double vision comes from one eye's optics. Get the cause diagnosed first.

How to cure double vision naturally?

There is no single natural cure, because double vision has many causes. The AAO says microvascular nerve palsy almost always gets better on its own in about 6 to 12 weeks. Other causes need glasses, prisms, exercise therapy for certain eye-teaming problems, medicine or surgery. Some causes are emergencies, so get checked first.

Is sudden double vision an emergency?

Treat it as one. MedlinePlus says to seek emergency care for double vision, even if it goes away. Call 911 if it comes with weakness, trouble speaking, a drooping eyelid, a severe headache, or follows a head injury. An AAO telephone triage guide for eye clinics puts sudden double vision in its urgent group, to be seen within 24 hours, and tells staff to err on the side of safety.

What is the difference between monocular and binocular double vision?

Binocular double vision goes away when either eye is covered, because the eyes are not aimed together. Monocular double vision stays when the other eye is covered and comes from one eye's optics, such as astigmatism, dry eye, cataract or keratoconus.

What exercises help double vision?

For convergence insufficiency, programs use vergence exercises, often with tools like the Brock string, set by a clinician. The right exercises, settings and dose depend on your diagnosis. Exercises are not used for every cause, so ask your eye care professional what fits.

Does vision therapy help double vision after a concussion?

For convergence insufficiency after a concussion, the 2026 CONCUSS trial found 88% of people were successful or improved after six weeks of office therapy, compared with 8% of those waiting to start. Any double vision after a head injury needs emergency care first.

Do prism glasses fix double vision?

Prisms align the two images and are widely used. In the published studies in the table above, 51% to 92% of patients had success or satisfaction, depending on the study and cause. The AAO notes prisms do not help every type of double vision.
